Historical Engineering Disasters and What Could Have Been Done to Prevent Them

Abstract:

If you have ever heard the story of how “the Kingdom was lost for want of a nail” in a horseshoe, you know that it is sometimes the littlest things that can cause the biggest disasters. In this presentation you will be introduced to some of the world’s largest engineering disasters. You will see how a seemingly small error, a part costing only a few cents or even a pen stroke, made the difference between success and failure. You will then work in a brainstorming session to probe and uncover some types of simple measures could have been taken to prevent the disaster or preclude future similar problems. The tools and techniques applied in this presentation are the kinds of things that you can immediately take back to your workplace and use to make improvements in your day to day activities.

Bio:

Mr. Haigh provides applied, tactical and strategic consulting to all levels of management from executives to developers. He has insight into the challenges faced and solutions needed by firms both large and small as they mature their project management and product and service development processes, methods, and tools.

Mr. Haigh has over 35 years in software development, IT network and infrastructure management, project management and the development and rollout of process improvement programs. He has held many positions within the software development lifecycle including Business Analyst, System Architect, Database Designer, Developer, Tester, Integrator, Technical Writer and Software Technical Support Specialist. Mr. Haigh has also provided a variety of management and support functions: Quality Assurance Manager; Software Configuration Manager (SCM); Software Engineering Process Group (SEPG) Lead; and Manager of Process and Quality. He also managed Network Operations for 10 years including administration of servers, databases, Email, telecom systems, LAN / WAN networks both internally and with the Internet and client networks using CMMI and ITIL principles and practices.

Mr. Haigh has experience in a variety of industries including government agencies (NASA, US Navy), Nuclear Power Generation, Transportation, Health Care, Telecommunications, Large Retail Operations, Software Application Development, and Service Providers of Internet services and their related consulting partners. He has presented at software process improvement conferences and is a Carnegie Mellon University Software Engineering Institute (CMU SEI) Certified CMMI Instructor. Mr. Haigh has extensive CMMI® based process development, implementation and SCAMPI appraisal team experience. He is also ITIL Foundation Certified and is a Six Sigma Black Belt.

Recommendation: The best way to turn onto Central Parkway is via from Perimeter Center West due to recent construction. Park in center section (not visitor section) of parking deck on your left as you enter 7000 Central Parkway. Take the parking deck elevator to floor 3 (lobby level) and follow the signs.
